Who is Preseli Cares for?
Preseli Cares is working across NE Pembrokeshire, initially we are engaging with and supporting people living in two areas, although anyone working, volunteering or providing care and support in NE Pembrokeshire is welcome to join us in Newport and its surrounding communities, and Crymych and its surrounding Communities

Preseli Cares will be working with these two communities to facilitate community led care, and solutions to social care issues. This includes people in receipt of social care, carers, paid and unpaid, local businesses, community groups and volunteers providing support and services. We welcome your input, and everyone is encouraged to be involved.

Become a Changemaker
INFORMATION FOR CHANGEMAKERS
Thanks to the National Lottery Community Fund Preseli Cares is a project helping communities to have voice and control over their care in North-east Pembrokeshire. PAVS is working in partnership with Cwmpas (formerly known as the Wales Co-operative Centre).
We are a `test and learn` project and over the next two years we aim to show that community-led care provides an affordable and viable solution to social care issues.
Preseli Cares
PAVS will work with people living in North-east Pembrokeshire, community groups, statutory partners, and private sector businesses to co-produce Preseli Cares.
Why get Involved?
Taking a co-productive approach means we recognise that everyone in the community has strengths and skills to share.
Simply put, co-production is about sharing power and responsibility and working together for our mutual benefit and well-being. This can’t be achieved without involving the very people and communities who use or may use services in the future. Co-production is a combination of the ‘learned’ expertise of professionals and the ‘lived’ experience of people in our communities.
We want to use the knowledge of professionals alongside the lived life experience of everybody in our communities. Initially we will be working in the Preseli GP Practice catchment area
If you are a person currently accessing care services or are a volunteer, a paid or unpaid carer, or are just living or working in our community, we encourage you to come and join us and be a Changemaker.
Together we can put community at the heart of social care.

Choose how to get involved
1 Become part of a community Project Development Group
We will be setting up project development groups in local communities.
These will be community forums where we will bring everyone together to find community-led solutions to social care issues that matter to you. These groups will meet regularly, and we will try to meet at a time and place to suit group members.
2 Become a Changemaker
You can help us to make care better for everyone by becoming a Changemaker. We want to find out about you and what you think the community could do to provide better care and support for each other.
Become a Changemaker by giving us some details about yourself and keeping us informed about how you are thinking and feeling as the project grows.
3 Sign up for regular updates
If you don’t want to sign up as a Changemaker or be involved in the project development groups, but do want to hear how we are progressing, you can sign up for our updates or visit the Preseli Cares project page on the Pembrokeshire Community Hub website where you can find more information on our progress. If you are not online, we will find other ways to keep you updated.

Community
Our focus is on achieving well-being and improving access to care and support for people, mobilising people’s skills and strengths within the local Preseli communities.

Collaboration
We aim to create better collaboration between community groups and local democratic structures, more opportunities for community-led service development, and more local activities to reduce isolation and strengthen our individual and community well-being.
